“Chicago Blackhawks® Cash” Second Chance Promotion
Official Rules & Procedures

1. The Illinois Lottery’s “Chicago Blackhawks® Cash” second chance promotion (hereinafter referred to as the "Promotion") will begin 12/28/2009, and will award two trips to a Chicago Blackhawks® away game in the 2010-2011 regular season.  The entry deadline for the promotion is 4:30 p.m. on May 28, 2010, and the prize drawing will occur on or about June 3, 2010.  Each prize package includes:
   
   
   • Round trip charter air for two on the Chicago Blackhawks® team plane
   • Lodging in the hotel the where the Chicago Blackhawks® stay (one room, one night, double occupancy)
   • Two Chicago Blackhawk® replica jerseys
   • Two tickets to the game
   • Transportation for two on the team bus to and from morning skate and the game
   • $500 cash

   Trips are subject to availability; therefore, neither the Illinois Lottery nor the Chicago Blackhawks® organization can guarantee that a particular game date or destination will be available.

   Each winner is responsible for their own transportation to and from the airport from which the team plane departs (Chicago O’Hare).

2. To enter the Promotion drawing, an individual must be at least 18 years old, meet the eligibility requirements of Section 6 below, and do the following:
   
   
   • Complete the backs of five (5) non-winning Chicago Blackhawks® Cash tickets with his or her name, address, and telephone number. Only one name per ticket, and the same name on all tickets constituting an entry. Preprinted address labels are acceptable and encouraged for legibility.
     
     
   • Place the completed ticket(s) in an envelope no larger than 4½” x 9½”;
     
     
   • Either:
     • Mail the completed entry to:

       OR
       
       
     • Hand deliver the entry to Lottery Central at 101 West Jefferson Street, Springfield, Illinois, clearly marked “Illinois Lottery Chicago Chicago Blackhawks ® Cash Promotion".
       
       

   Only One (1) Entry Per Envelope.  If an entry envelope contains more than five (5) non-winning Chicago Blackhawks® Cash instant tickets, it will constitute only ONE entry.  Players may submit as many entries as they wish, but may win only one prize in the Promotion.

3. Entries will be disqualified if:
• Less than five (5) non-winning Illinois Lottery Chicago Blackhawks® Cash tickets bearing the entrant’s name are contained in the entry envelope;
• Entrant name, address and telephone number is illegible or incomplete to the extent the entrant cannot be contacted;
• More than one name appears on the backs of the non-winning tickets submitted in the entry envelope;
• Entry envelope is larger than 4 1/2” x 9 1/2”;
• Entries are not received by 4:30 p.m. on May 28, 2010;
• The entrant is less than 18 years old or is otherwise ineligible to participate in the Promotion by virtue of these rules, the Illinois Lottery Law, or any rules and regulations promulgated pursuant thereto. 

The Illinois Lottery is not responsible for lost, mutilated, illegible, misdirected or delayed entries.

4. The drawing to select the two Promotion winners will be conducted by Lottery officials, in the presence of an auditor, pursuant to written drawing procedures. The winners will be determined by officials of the Illinois Lottery, whose decisions are final.  Winners may be notified in person by Lottery officials, by telephone, or by overnight express letter.  If a winner is found to be ineligible to participate in the Promotion, or otherwise forfeits the prize, another winner may be selected.  Promotion winners will be required to complete, sign and return an Illinois Lottery claim form in order to receive their prize.
   
   
5. The Illinois Lottery has elected to pay required state withholding amounts associated with the fair market value of the prize package, which has been determined to be $3,600.00.  The amount of tax withholding paid by the Lottery on behalf of the winner is required to be added to the fair market value of the prize for tax reporting purposes.  End of year federal and state income taxes and any other taxes are the sole responsibility of the winner.
   
   
6. The Promotion is open to persons 18 years of age or older, except members of the Lottery Control Board; officers and employees of the Illinois Department of Revenue; officers and employees of the Illinois Lottery’s advertising, public relations, and sales promotion agencies assigned to the Lottery’s account; officers and employees of audit firms performing work for the Illinois Lottery; and certain other contractors of the Illinois Lottery.
   
   
7. The odds of winning are dependent upon the number of entries received for the drawing.
   
   
8. Tickets, transactions and winners are subject to all Lottery procedures, rules, directives, State law and any lawful changes thereof.
   
   
9. In the event a winner is unable or unwilling to participate in the travel component of the prize, the winner may substitute another individual as the primary traveler, provided that the individual is at least 18 years old. The winner must notify the Illinois Lottery Claims office of the name of the primary traveler within 7 business days or else the travel component of the prize will be forfeited.  In the event of traveler substitution, the winner will continue to be recognized as the prize recipient for reporting purposes.
   
   
10. This Promotion is being conducted solely by the Illinois Lottery.
    
    
11. Entrants agree to hold harmless and indemnify the Chicago Blackhawk Hockey Team, Inc., the National Hockey League and its Member Teams and each of their respective parents, subsidiaries, affiliates, directors, officers, partners, members, employees and agents against any and all liability, damages or causes of action (however named or described), with respect to or arising out of the entrant’s participation in the promotion, the receipt or use of the prize awarded or the use of the winner’s name, photograph and/or likeness as authorized by the winner in claiming their prize.
    
    
12. NO CASH SUBSTITUTION FOR THE PRIZE IS ALLOWED.
